FOR SPACE LOCATED AT 4216 NORTH BROADWAY, TO BE USED AS A COVID-19 VACCINATION SITE, FOR A SIX-MONTH TERM BEGINNING FROM COMMENCEMENT DATE AND ENDING AUGUST 31, 2021 WITH THE OPTION TO EXTEND MONTH TO MONTH WITH A RENT AMOUNT OF $5,700.00 PER MONTH:
