A Resolution authorizing the Mayor to accept a proposal from Ochs, Inc., underwritten by the Madison National Life Insurance Company, Inc., to provide employee long-term disability insurance coverage for a period of three years at an annual estimated cost of $170,000.00 and to execute a contract and application for insurance consistent with the proposal. (Requested by Employee Benefits & Risk Management)
