A Resolution authorizing the Mayor to accept a proposal from Ochs, Inc., underwritten by the Minnesota Life Insurance Company, to provide employee and dependent life and AD&D insurance coverage for a period of three years at an annual premium not to exceed $125,000.00 and to execute a contract and application for insurance consistent with the proposal. (Requested by Employee Benefits & Risk Management)
