Amend health center agreement
A Resolution authorizing the Mayor to execute an amendment to Agreement No. C-17-0060 with Premise Health Employers Solutions, LLC, for operation and management of the City's employer-sponsored health center and field offices, increasing the budget for the final year of the agreement by an amount not to exceed $40,000.00, to allow for overtime required due to additional COVID-19 duties, for a total cost for the fifth and final year of the agreement not to exceed $2,334,977.00.(Requested by Employee Benefits & Risk Management Department)
